Student Well-Being Survey
If you want to check in, at any time, on your students’ well-being, here’s a quick way to use a GoogleForm and a few questions to help. Although I created this at the start of the Covid-19 school closing, it could be useful throughout the school year.
As an important note, if any of my students self-reported the RED light, I went directly to our counselors and administration.
Also, if you turn the FORM into a QUIZ, you’ll be able to respond to your students as well! It’s another great option.
